Health App & Wearable Feedback
Tracking accuracy, habit fit, and whether the data changed any behaviour.
4 questions · 1 page · about 2 minutes to answer · fully editable after you start
Health tech is judged on a brutal question its dashboards avoid: did anything about your health actually change? This survey asks it directly, alongside the experience layer – tracking accuracy trust, notification helpfulness versus nagging, battery-and-wear comfort – and the abandoned-features map every health product accumulates.
The behaviour-change answers separate engagement from impact; a step counter opened daily that changed nothing is retention without health, and the roadmap should know.
