Good boarding recreates home in the pet's own terms, and that takes information: feeding routines and amounts, medications with timing, walk and play habits, quirks and fears, the owner's vet, and treatment consent with a spending cap. This check-in form collects it per stay, with vaccination-record upload attached.
The treatment-consent-and-cap question saves the 2am dilemma: staff act fast, owners stay in control, and both are documented.
