Summer Camp Registration
Camper details, sessions, swim level, health flags, and the signed consents.
7 questions · 1 page · about 4 minutes to answer · fully editable after you start
Camp registration must capture safety-critical details without exhausting parents. This form does the full intake: camper and sessions wanted, swim ability honestly banded (the waterfront's most important field), health and dietary needs, emergency contacts, and the consent set – activities, photos, medical treatment – signed once, dated, and stored per camper.
Session capacity pairs naturally with collector response limits: registration closes itself when the bus is full.
