Parent Observation Sharing Form
Two-way records: what parents see at home, for the setting's learning journal.
5 questions · 1 page · about 4 minutes to answer · fully editable after you start
Early-years learning journals are richer when home observations flow in: the first unaided steps that happened on Saturday, the new words, the fascinations. This sharing form invites parents to contribute what carers cannot see – moments, new abilities, current obsessions, and questions – keeping the record genuinely two-way.
Settings using it report a compounding effect: parents who contribute read the journal more, and children whose journals span home and setting get more attuned care in both.
