Blanket you-may-be-filmed signage satisfies nobody anymore. This consent form does it properly: uses named specifically (website, social, sponsor materials), the genuine opt-out with its practical meaning explained, minors handled through guardians, and the signed record with date.
Naming the uses individually is the point. Consent to a conference recap video is not consent to a sponsor's advertising, and a matrix that separates them gives attendees a real choice while giving you a defensible record of exactly what each person agreed to – which a single yes-or-no checkbox never does.
Events collecting real consent also get better photos: subjects who chose to be photographed look like it. Keep the signed records for as long as you intend to use the imagery, not just for the duration of the event, and make sure whoever edits the gallery afterwards can actually see who opted out – consent collected properly and then ignored at the editing stage is worse than not asking.
