Facility Use Request
Community bookings of your space: purpose, date, needs, and agreement to house rules.
6 questions · 1 page · about 2 minutes to answer · fully editable after you start
Buildings serve mission when the booking process is orderly: who wants the space, for what, when, with what needs, under whose responsibility. This request form manages community use – internal groups and external hirers alike – with purpose, date and duration, setup and equipment needs, expected numbers, and house-rules agreement.
The purpose field protects the community's discernment: knowing what happens in the building is stewardship, not gatekeeping, and the form asks it plainly.
