This end-of-project form collects the client's verdict on the freelance fundamentals – brief understanding, communication, deadline reliability, quality – the future-work intent, and the testimonial request framed so busy clients can say yes in one minute.
The would-refer question converts satisfaction into pipeline: clients rarely volunteer referrals but often agree when asked at the moment of delivered value.
