Machine Training Sign-off
Competency confirmed and signed: operations, safety devices, and abnormal situations.
5 questions · 1 page · about 2 minutes to answer · fully editable after you start
Training records protect people first and audits second. This sign-off form confirms operator competency item by item – normal operation, guards and interlocks, abnormal-situation response, lockout basics – with a self-rated confidence check and both signatures, trainee and trainer, dated for the record.
The confidence self-rating catches what checkbox training misses: an operator who passed everything but rates abnormal-situations low needs supervised hours, not a filed form.
