Tryout days run on paper chaos or on this form: player details, positions in preference order, experience honestly stated for fair grouping, season availability, and the medical flags coaches must know before drills start.
Collect it before the day rather than at the gate. A coach standing in a car park with a clipboard is not reading medical notes carefully, and the whole value of asking about injuries is lost if the answers are still in a pile when the first drill starts.
The availability question saves the classic waste: the standout who cannot make half the season is better known before the roster forms around them. Use the experience answer for grouping rather than for cutting – honest self-assessment only keeps working if players do not learn that admitting inexperience costs them a place, and a tryout that groups fairly gets far better information than one where everyone claims to be advanced.
