Collect benefits choices without the email spreadsheet round
Enrolment season means a spreadsheet emailed to 300 people, forty replies with the columns rearranged, and a week of HR reconciling typos before the provider deadline.
The short way: One structured form — plan choices, dependent details, validated fields — that lands as clean rows, exportable in exactly the provider's shape.
Ready-made templates for this

Type this — that’s the whole job

No setup, no tutorial. The words above go with you — review what the AI builds, then share the link.
How it works — three steps
- 1
List your plans and required fields; the AI builds the form with the right input types.
- 2
Invite everyone from your contact list; reminders chase only the stragglers.
- 3
Export to Excel in provider-ready columns when the window closes.
What’s doing the work
Multiple textboxes
Dependent name/DOB pairs as labelled rows, not a free-text field HR must re-parse.
Validation rules
A contribution percentage outside 0–100 is refused at entry, not discovered at the provider.
Invitation trackingPro
See exactly who hasn't enrolled with a week to go, and remind only them.
Questions people ask
Is a survey tool appropriate for HR data like this?
With access controls, retention settings and no third-party AI touching responses by default — yes, and considerably safer than the emailed spreadsheet it replaces.
Can employees change an answer after submitting?
Not once it is sent — a submitted response can't be changed. Turn on 'Ask for confirmation before the final submit' on the Options tab so everyone checks their choices first; if someone still needs a change, allow multiple responses on the link and delete their earlier one on the Responses tab.
