Caregiver Support Survey
Check on the people doing the caring: burden, respite, and what support would help.
4 questions · 1 page · about 2 minutes to answer · fully editable after you start
Family caregivers hold up half of healthcare and are the last to report their own strain. This survey asks caregivers directly about hours of care, their own health and sleep, financial pressure, and which supports – respite, training, counselling, or simply information – would genuinely lighten the load.
For care organisations, the aggregate results justify support programmes; for individual services, a high-burden response is a flag to reach out before the caregiver becomes the second patient.
Replace [local emergency number, e.g. 000] and [local crisis line, e.g. Lifeline on 13 11 14] in its opening note with the services where your respondents live, before you share it: the numbers shown are Australia's. The Collect tab lists both until they are changed.
