Course feedback is usually collected on the last day, when the people best placed to judge the material are also the most eager to leave. This template is short enough to complete honestly in that window, and structured so the results distinguish between teaching, content and logistics — three different problems with three different owners.
It asks whether the level was right, whether the pace worked and whether the workload was fair, each on a balanced scale with "about right" in the middle. A grid rates the course materials, the teaching, feedback on work and the practical arrangements, and one question asks whether the learner can now do something they could not do before. That is the only question that measures the course rather than the experience of attending it.
The recommend question gives you a score to compare across courses. The last three questions are open and deliberately broad: what helped most, what to add or remove, and anything else. In practice they are where you learn that the room was too cold, the prerequisites were wrong, or a single exercise did most of the work.
