A course evaluation only helps if students believe someone reads it – and if it asks about things a teaching team can actually change. This template opens with the context that makes the ratings readable: how much of the teaching the student attended, why they took the course and how many hours a week it took. It then asks for agreement with specific statements about the course – clear learning outcomes, organisation, assessment that tests what was taught, a fair workload, timely feedback and useful resources – and about the teaching, with a Not applicable choice for anything a student did not experience.
The overall item is the standard satisfaction statement used in national course experience surveys, so results can sit beside those benchmarks, and the two open questions are the ones that produce real syllabus changes: what helped you learn most, and what one change would most improve the course. Run it anonymously in the final two weeks while the course is fresh but before grades colour the answers. Comparing the same course across terms shows whether changes actually landed.
