Migrations fail on the dependencies nobody wrote down. This readiness survey collects each system owner's declaration before cutover: data quality confidence, known integrations and consumers, acceptable downtime windows, and the risk they are most worried about – creating the risk register from the people who actually know.
Run it three to four weeks before cutover with named responses. The most-worried-about answers deserve a line each in the migration plan; they are usually right.
