A trainer's first session should start with movement, not paperwork. This intake collects the essentials beforehand: training goals in the client's own words, exercise history, the PAR-Q health screening questions that flag when medical clearance is needed, and a signed declaration with the date.

Any yes on the screening questions routes to a physician-clearance conversation before programming begins – the standard of care that protects client and trainer alike. The signature is drawn on any device and stored with the response.

Replace [local emergency number, e.g. 000] in the health-check note with the emergency number where your clients live, before you share it: 000 is Australia's. The Collect tab lists it until it is changed.
