Students rarely raise a hand to say they are struggling; a short anonymous check-in lowers the cost of telling someone. This survey asks about stress levels, sleep, sense of belonging, and – most actionably – whether students know where to turn when things get heavy.
Run it monthly in anonymous mode and watch cohort-level trends rather than chasing individuals. The know-where-to-get-help question is your intervention lever: it is the one number a single awareness campaign can move within a term.
Replace [local emergency number, e.g. 000] and [local crisis line, e.g. Lifeline on 13 11 14] in its opening note with the services where your respondents live, before you share it: the numbers shown are Australia's. The Collect tab lists both until they are changed.
