Toolbox Talk Record
Topic covered, questions raised, and attendance signed – the five-minute record.
6 questions · 1 page · about 5 minutes to answer · fully editable after you start
Toolbox talks count only if recorded, and recording only works if it is quick. This form logs the talk – topic, key points, site-specific hazards raised – and captures each attendee's signature on their own phone or the supervisor's, replacing the rain-soaked signature sheet with a timestamped record.
The questions-raised field is the underused gold: crews flag the hazards the method statement missed, right there, on record.
