Usability Test Follow-up Survey
Post-session ratings that quantify what the moderator observed.
5 questions · 1 page · about 2 minutes to answer · fully editable after you start
A usability session produces rich observation and zero numbers; this follow-up adds the numbers. Completed by the participant right after the tasks, it captures perceived difficulty, confidence, and the single-ease ratings that let you compare sessions and iterations statistically.
The would-use-unassisted question is the honest one: participants who politely completed every task will still tell you here that they would not trust themselves alone with the product. Ten sessions with this attached become a trackable usability score.
