Volunteers leave quietly. They rarely complain and rarely resign — they simply stop being available, and by then the reason is difficult to recover. This template asks the questions that surface those reasons while the person is still involved: whether the role matched what they expected, whether the training was enough, whether they felt their time was used well, and whether they felt appreciated, knew who to ask, felt safe and welcome, and could see the difference they make.

"Was your time used well" is the one to watch. Volunteers are giving you the scarcest thing they have, and the most common cause of quiet attrition is not difficulty or conflict — it is standing around with nothing to do after making the effort to turn up. The likelihood of still volunteering in six months turns those answers into an early warning.

Keep it short and run it after a specific shift or event rather than annually. Feedback tied to a particular day produces specific answers; feedback about volunteering in general produces polite ones.
