Burnout Risk Check-in

An anonymous early-warning read on exhaustion, detachment, and recovery.

4 questions · 1 page · about 2 minutes to answer · fully editable after you start

Burnout is measurable long before it is visible: exhaustion that rest stops fixing, growing cynicism about work that used to matter, effectiveness sliding despite longer hours. This anonymous check-in reads those three classic dimensions plus the recovery question – whether evenings and weekends actually restore anything.

Run it quarterly at team granularity, never individual. A team trending down on recovery is a workload conversation for its leader this month, not a resilience webinar next quarter.

Replace [local emergency number, e.g. 000] and [local crisis line, e.g. Lifeline on 13 11 14] in its opening note with the services where your respondents live, before you share it: the numbers shown are Australia's. The Collect tab lists both until they are changed.

Honestly, how are you?

This check-in asks how work has been affecting your energy and wellbeing over the past month. Answer for yourself, as honestly as you can. It takes about 2 minutes.

This check-in is not a crisis service. If you are in immediate danger, call [local emergency number, e.g. 000]. For crisis support at any time, call [local crisis line, e.g. Lifeline on 13 11 14].
