Most training feedback measures whether people enjoyed the session; this one measures whether it will change how they work. It covers the first three levels of the familiar training-evaluation model: reaction (relevance, trainer, pace, materials and the chance to practise, each with a Not applicable choice), learning (a before-and-after self-rating of knowledge, which shows the shift more honestly than a single after-score), and intended behaviour (whether people can apply it, and whether they will have the time and support to).
The open questions ask what they will do differently next week and what might get in the way – the second is usually the more useful, because it names the obstacle before it wins. The standard recommend question closes it. Send it twice – immediately after the session, and again 30 days later asking what actually changed. The gap between intention and application is your real training return, and it is usually a follow-up problem, not a content problem.
