This evaluation form has each evaluator score a vendor independently – capability fit, commercial terms, support model, risk factors – with required evidence for extreme scores, before any group discussion anchors the room.
Run one response per evaluator per vendor and compare in the decision meeting. The pattern of independent scores tells you more than their average: consensus is a decision, divergence is an agenda.
