Accessibility handled at the door is accessibility failed. This pre-event form asks registrants what they need in time to arrange it: mobility and seating, communication support (captions, interpreters, hearing loops), sensory considerations, dietary requirements, and companion arrangements.
Send it with the registration confirmation and set your own internal deadline well before the venue's. Interpreters, captioners, and accessible transport are booked weeks ahead, and a need disclosed in good time that you then could not arrange is a worse outcome than never having asked.
The free-text catch-all matters most: real needs are specific, and the organiser who reads them buys goodwill no ramp photo earns. Name a person who reads the responses and make sure they can actually act – a form that collects requirements into a spreadsheet nobody owns is the same failure as not asking, with the added insult of having invited someone to explain themselves first. Confirm back to each registrant what has been arranged, so nobody arrives hoping.
