Offboarding Handover Form

Capture the leaver's knowledge before it walks: systems, contacts, and the undocumented.

6 questions · 1 page · about 5 minutes to answer · fully editable after you start

Every departure exports undocumented knowledge, and the export window is two weeks. This handover form has the leaver inventory what only they know: systems and access they hold, in-flight work and its state, relationships and contacts that need warm handoffs, and the deliberately open question – what will break when you leave that nobody realises.

The will-break answers are routinely worth the whole form: leavers know exactly where the bodies are buried and, asked respectfully on the way out, will draw the map.
