Reviews go better when the employee's own account arrives first. This self-assessment asks for the year's proudest work, the miss that taught the most, a skills self-rating, and – the question managers value most – what support was missing. It frames the review conversation around evidence instead of recall.
Send it two weeks before review meetings and attach responses to the review record. The support-needed answers double as a training-needs analysis across the whole company.
