The first month decides whether a new hire commits or quietly reopens their job search. This 30-day check-in measures the four things that predict early attrition: role clarity, working equipment and access, manager one-to-ones, and whether the job matches what was promised in the interview. A satisfaction grid covers each part of onboarding – equipment, training, manager, team and the pay-and-policy paperwork – with a Not applicable choice for anything that has not happened yet.
Three open questions ask what has helped, what has been confusing and what they still need, and the last question is the standard employee Net Promoter item so the 30-day reading lines up with your other people surveys. The honest-doubts questions are deliberately anonymous-friendly: the intro says so, a web link made for this survey starts with anonymous responses on, and new hires will tell you what they would never say in a hallway. Repeat at 90 days with the same wording to see whether the gaps closed.
