Free Legacy System Assessment template
Software and IT firms: what an old system does, its technology, age, users, source code, concerns, timing and budget.
12 questions · 5 pages · about 4 minutes to answer · fully editable after you start
Modernising old business software starts with knowing what is there. This assessment is the questionnaire Thisys Technologies built for its own website, made general: what the system does, what it is built with (PHP, Classic ASP, .NET Framework, VB, Java, Delphi, Access or Excel), how old it is and how many people use it, whether the source code is in hand, how many separate systems are involved (with a follow-up on which kinds, asked only when there is more than one), the three biggest concerns, when action is needed and the budget set.
Replace [Your firm, and the systems you modernise] on the opening page and [two business days] in the closing note. It never asks for passwords or access to the system: an assessment needs a description, not the keys.

Your system
This assessment asks what your old system does, what it is built with and what worries you about it, so a first call can be about the options. It takes about 4 minutes.
