Free Client Intake and Assessment Questionnaire template
Qualify a new client before the first call: the need, who is involved, concerns, timing, budget band and consent.
11 questions · 5 pages · about 5 minutes to answer · fully editable after you start
A contact form says too little. A name, an email address and "please call me" means every enquiry gets the same discovery call, and the ones that were never a fit cost the same hour as the ones that were. This questionnaire asks first: what the client needs in their own words, how far along they are, who else is involved (asked only when more than one team is affected), the three things that worry them most, when they want to start and the budget band they have in mind.
Replace [What you do, and who you do it for] on the opening page with your own line, and [two business days] with the reply time you can keep. Share the link from your website's contact page or an advertisement: the campaign tags and ad clicks in the link are kept with each answer, so you can see which sources bring the clients worth calling.
